We take the time to create a personalized treatment for each patient and we offer a wide range of services, from family and cosmetic dentistry to periodontics and implant dentistry. It is our goal to make your visit with us an extremely pleasant one and to send you home with a bright, beautiful and healthy smile.

Patient Forms

Below are the Medical History and Privacy Policy forms you’ll need to complete on your first visit. You can expedite matters by downloading these forms, completing them at home and handing them to our staff when you arrive for your initial visit to our office.

Payment Options

We accept VISA, MasterCard, Discover, and American Express credit cards as well as personal checks. We also offer no interest or low interest financing through CareCredit and Lending Club. Additionally, we accept most dental insurance plans. To find out if we accept your plan, please call and speak to our office manager, Cheryl, who will be more than happy to assist you.

What to Expect

On your first visit, Dr. Schulman will review your medical and dental histories and evaluate your oral health. We will take digital x-rays of your teeth and explain to you whether or not you will require any dental treatment. We will be more than happy to answer any questions you might have. In most cases, we will perform a cleaning of your teeth on this first visit.
